Creating Least Restrictive Care and Support Packages: A Holistic Approach

Seco Support, in the pursuit of supporting individuals with learning disabilities and autism to lead fulfilling lives within their communities, recognises that the challenge lies in creating care and support packages that are both least restrictive and ensure safety. In alignment with the NSHE-Building the Right Support 2015 vision, which emphasizes the right to live satisfying lives with dignity and respect, we delve into the enablers and strategies that contribute to the creation of effective community placements.

Children, young people and adults with a learning disability and/or autism have the right to the same opportunities as anyone else to live satisfying and valued lives, and to be treated with dignity and respect.

They should have a home within their community, be able to develop and maintain relationships, and get the support they need to live healthy, safe and rewarding lives.”

Understanding the Person: A Comprehensive Approach

“Get to really know the person” is not just a phrase Seco Support throw about; it’s the foundation of providing tailored support. Utilising at least a year of data allows us to examine setting events, triggers, behaviours, consequences, and functions. It involves understanding the person’s current environment and identifying what environment is needed for them to thrive. Documentation may focus on negatives, but building a relationship with the individual and their family helps reveal a more comprehensive picture.

Transition Planning: A Clear and Achievable Roadmap

Transitioning from hospital to the community demands a clear and achievable plan. By setting up regular transition meetings, starting from the move date and working backwards, and ensuring the involvement and agreement of all stakeholders, a solid foundation is laid. The goal is to have everyone sign up to the same objective, fostering collaboration and shared responsibility.

Service Design: Improving Quality of Life

Service design plays a pivotal role in improving the quality of life for the people we support. Utilising tools such as the BILD QoL tool and following The Care Act ensures detailed support plans and risk assessments are in place. The PERMA model, skills teaching, and positive risk-taking align with the person’s preferences. A bespoke service and team, involving the person, their family, and professionals, contribute to a personalised and enriching experience.

The Reach Standards play a vital role in the statements that need to be adhered to.

Positive Risk Taking: Balancing Independence and Safety

Recognising different types of risk, careful assessment and planning, staff training, and close monitoring are essential components of positive risk-taking. Balancing creativity with safety, having contingency plans, and ensuring everyone is on board contribute to an environment where individuals can push boundaries within safe limits.

Trauma-Informed Practice: Understanding and Addressing Trauma

Acknowledging trauma and its impact is crucial. Trauma-informed practice recognizes the potential impact of trauma on an individual’s development. Understanding and addressing trauma helps in providing more sensitive and effective support.

Joint Working and Training: Collaboration for Success

Collaboration is key to complex support and risk management. Joint working ensures everyone is moving in the same direction, with an advocate for the person. Bespoke training, focusing on autism education, support plans, and risk assessments, equips the team to fulfil their goal of supporting the person to live a life they choose.

Service Delivery and Review: An Organic and Adaptive Approach

Service delivery should be organic, changing in line with the person’s development. Regular MDT reviews, key performance indicators aligned with skills teaching and PERMA, and goal setting involving the person and their family contribute to a dynamic and effective support structure.

The journey from hospital to the community is complex, requiring a holistic and person-centred approach. Communication, information sharing, creativity, and meticulous planning are vital in ensuring a successful transition that respects the individual’s right to lead a meaningful and rewarding life.
